| You know resident #22 as Esther Pettinga, always delightful and one of your most stable residents that you formally check on only monthly due to her low Health Burden of 8. The housekeepers have gradually been seeing a change in Esther, however. Her room is becoming messy, open bottles of vitamins and other non-prescription medicines have been found lying around the apartment, and she has occasionally been found wandering in the hallways at night and unable to find her way back to her apartment. Of greatest concern is that Esther admitted to your housekeeper that she fell yesterday and had some difficulty getting up from the floor. |

You review Esther’s Health Profile and see that she only takes 2 prescription medicines, so it is unlikely a medication reaction is causing Esther’s change in behavior. You then see that Esther carries a diagnosis of dementia (which had been only very mild), and you’re concerned this may be worsening and causing her decline. Your nurse visits Esther and finds no obvious injuries from her recent fall. You speak with Esther’s family, and a doctor’s appointment is made which confirms your suspicions. A personal care service is hired to regularly check on Esther and assist her with her needs, and her family will check on her more often. Esther’s daughter is very appreciative for your knowledge and care of her mother and feels very comfortable keeping her mother in your facility rather than transferring her to a higher level of care. |
